An incident occurred on November 19, 2007, in Chilliwack, B.C., involving a hit and run on Yale Road at Airport Road. The affected person arrived at a rental centre to return a wood chipper. A report was made to the RCMP Detachment, and a conversation about the hit and run driver was overheard by the affected person. The affected person attempted to go upstairs into the office space but was physically prevented from doing so by the store owner. The store owner instructed an employee to call the police. The police were called to report a disturbance, and the store owner remained on the stairs to prevent the affected person from moving up. The police were informed that assistance was still needed, and a request was made for members to attend.

On a date in late 2007, in Vernon, British Columbia, an individual who suffered from a mental illness was shot and killed by members of the Royal Canadian Mounted Police. A mental health warrant had been issued to apprehend and return the individual to the hospital for psychiatric treatment. The RCMP members mistakenly believed the individual was in the process of fatally harming themselves, and they breached the door to the individual's apartment and entered with their guns drawn, planning to apprehend the individual at gunpoint. The individual was not acting rationally and engaged the RCMP members with two knives in their hands. A conducted energy weapon was used. The Commission reviewed the facts surrounding the events, including the RCMP's response to the mental health warrant, risk assessment, and plan to apprehend the individual, as well as the tactical entry into and search of the individual's apartment, and the shooting. The Commission also examined the initial investigative response, the criminal investigation, and the timeliness of the investigation. The Commission commented on the RCMP's review of member conduct and the public complaint investigation.

A shooting death occurred in a mall in a British Columbia community. The subject officer, a member of the local RCMP, responded to a break-in alarm. The officer observed the affected person running from the mall and pursued them in a police vehicle. The officer exited the vehicle near a community facility and continued to pursue the affected person on foot, yelling commands to stop. The affected person eventually stopped and turned around. The incident led to a complaint filed by the Chair of the Commission for Public Complaints Against the RCMP, which was later supplemented with additional allegations.

Chair-Initiated Public Complaint: In-custody death of the affected person, in October 2007, involving the use of a conducted energy weapon at the Vancouver International Airport. As Chair of the Commission for Public Complaints Against the RCMP, a complaint is being initiated into the conduct of those unidentified members present at, or engaged in, the arrest at the Vancouver International Airport, and the adequacy of the subsequent investigation. The complaint is being initiated with the appreciation that: the agency has deployed an investigation team to investigate the incident; an Independent Officer Review has been initiated to assess the conduct of involved members, related policies, procedures and training; and that no decision has been taken in respect of the laying of charges. It is not the intention to prejudice any of the above investigations. However, progress will be closely monitored so that a timely response to this complaint can be ensured. A representative will be in attendance at the inquest once the date and location are announced.

The Commission's Final Report after Commissioner's Response Regarding the shooting death in Buick Creek, B.C. The Chairperson-Initiated Complaint [1] On September 26, 2009, the Fort St. John RCMP in British Columbia was contacted by a member of the public with respect to an incident involving a person and their family. It was reported that the person stopped a vehicle containing their spouse and children, who were on their way to attend a wedding in another town, and shot out two of the tires with a rifle. The driver of the vehicle was able to proceed to a nearby residence, but the person followed them and shot out the remaining two tires. The person eventually returned to their residence following several failed attempts to speak with their family and to get them to return home. Their family travelled on to the wedding and remained out of town until the matter could be resolved, on the advice of the RCMP. Attempts were made by members of the Fort St. John Detachment to speak with the person, but they were not cooperative and insisted that their family be returned to them. [2] The RCMP North District Emergency Response Team (NDERT) was ultimately activated.

The agency's final report regarding a motor vehicle incident in a city in British Columbia. On a date in 2012, members of the traffic section were conducting a targeted traffic enforcement operation in response to complaints from the public of speeding and aggressive driving in the area. The members involved were also using an Automated License Plate Recognition system to monitor passing traffic. Members attempted to stop a vehicle identified by the system as being associated with a prohibited driver. The vehicle failed to stop and fled. Two unmarked vehicles attempted to catch up to the fleeing vehicle but discontinued their attempts shortly thereafter. Within moments, the fleeing vehicle failed to stop at a stop sign and collided with a semi-trailer truck. This collision caused the trailer to collide with a third vehicle driven by the affected person. The affected person sustained fatal injuries as a result of the collision and died at the scene. The incident was reviewed under the Royal Canadian Mounted Police Act Subsection 45.72(2).

The complaint concerned the actions of the subject officer during a planned liquor enforcement operation on the evening of a date in late 2005. The affected person was arrested for "open liquor" and obstruction, and was later transported to the RCMP detachment. A public interest investigation was conducted into the use of a conducted energy weapon by a member of the RCMP in the Northwest Territories. The incident occurred on March 13, 2007, at a youth facility where a female inmate was being restrained by youth officers. The officer attended the scene and used a conducted energy weapon.
