The Independent Investigations Office (IIO) of British Columbia conducted an investigation into an incident involving the serious injury of an adult in the city of West Vancouver. The affected person sustained a serious injury to their head, allegedly while being transported by officers to the police department. The incident occurred in mid-2013. The IIO asserted jurisdiction pursuant to Part 11 of the Police Act, as the incident constituted "serious harm" as defined under that legislation. Upon conclusion of the investigation, the Chief Civilian Director (CCD) reviewed the matter as required under s.38.11 of the Police Act to determine whether an officer may have committed an offence under any enactment, including an enactment of Canada or another province. This public report has been issued pursuant to s.38.121 of the Police Act, indicating that the matter was not reported to Crown Counsel, and providing a summary of the results of the investigation and the reasoning underlying that decision.
