The Canadian Forces National Investigation Service Western Region (CFNIS WR) investigated two sexual assault allegations made in late 2021 against a Canadian Forces officer posted to a base in Saskatchewan. Following a short investigation, the officer was arrested and charged with two counts of sexual assault; they died by suicide in early 2022. The Military Police Complaints Commission conducted a Public Interest Investigation and found the CFNIS WR investigation was inadequate, marked by undue haste, confirmation bias, and tunnel vision. The CFPM declined to implement any of the recommendations issued by the MPCC Chairperson in the final report dated in 2026.
